There is an uncomfortable contradiction inside many physician households.
Parents spend years sacrificing to create stability and opportunity for their children. They endure medical school, residency, long call schedules, missed holidays, and countless nights when medicine gets the best hours of the day. Then, when the financial security finally arrives, so does another fear:
Have we given our children a better life—or made life so comfortable that they are unprepared to manage it?
That question is usually framed as a concern about entitlement. But entitlement is rarely the entire story.
Sometimes children expect more because no one has clearly taught them what enough looks like. Sometimes they have never been given genuine ownership or meaningful responsibility. And sometimes, particularly in medical families, money starts doing work that money was never meant to do.
A gift becomes an apology for another missed event. An expensive vacation is expected to compensate for months of disconnection. A parent says yes because guilt makes setting a boundary feel like one more disappointment.
But money cannot replace time, and opportunity cannot replace connection.

Julia’s work is not really about teaching children how to budget. It is about helping families raise children who can handle responsibility, make thoughtful decisions, tolerate limits, recover from mistakes, and understand that privilege comes with stewardship.
When Gifts Become a Substitute for Presence
Physician parents often miss moments they desperately want to be part of. The guilt attached to those absences can make it tempting to compensate with gifts, experiences, or an automatic yes.
Julia offers another path.
A voice message from the hospital saying, “I saw something that made me think of you,” may create more connection than another airport souvenir. Explaining why a physician is on call, who may need them, and what that responsibility means gives children context. It allows medicine to be discussed honestly without pretending that the family never feels its cost.
As Julia says:
“Conversation leads to connection, not cash. Conversation is the currency of connection.”
Children do not only need more access to their parents’ resources. They need access to their parents’ thinking, values, decisions, and humanity.
Entitlement May Be the Symptom
It is easy to call a child entitled when they expect a car, the newest shoes, or another expensive experience. It is harder to ask what the adults may have unintentionally taught them to expect.
Julia identifies three common gaps that keep children from becoming financially mature:
Skills. Ownership. Expectations.
Children need more than financial information. They need opportunities to practice making decisions while the consequences are still manageable. They need age-appropriate responsibilities, clear expectations, and permission to make small mistakes without a parent immediately stepping in to erase the discomfort.
Confidence does not come from getting everything right. It comes from learning that a mistake can be handled.
Generosity Changes the Conversation
Julia describes generosity as the antidote to entitlement because it teaches children that what they have is not exclusively for their own consumption.
But generosity is more meaningful when children participate in it. Rather than simply telling them where they must give, families can ask what matters to them, whom they want to support, and why.
This is not about making children feel ashamed of privilege. It is about teaching them how to steward it.
What Are You Actually Leaving Behind?
Families spend considerable time preparing wills, trusts, investments, and estate plans. Far fewer spend the same amount of energy preparing their children to manage what they may one day receive.
Julia’s Family Constitution framework helps families identify what they value, how those values appear in everyday decisions, and what expectations guide the household. Businesses create mission statements and operating principles. Families should not leave their most important values to chance.
The real question is not simply how much children will inherit.
It is whether what parents leave will become a blessing or a burden.
Julia captures the difference beautifully:
“That’s the legacy I’m not leaving to them. That’s the legacy I’m leaving in them.”
For physician families, that distinction matters. Medicine may provide resources and opportunities, but it is the culture created inside the home that teaches children what those opportunities mean.
